Vasectomy reversal: microsurgical vasovasostomy
9 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.
Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

- 01
Couple assessment, examination
- The couple are assessed together, the female partner's age and fertility included, and sperm retrieval with ICSI is discussed as the alternative.
- Each vasectomy site is felt: the gap, and a granuloma on the testicular end.
- 02
Incision on the median raphe
- A vertical incision on the upper median raphe, deepened through the dartos to the cords.
- One incision reaches both vasa.
- 03
Delivery of the vas and its vasectomy site
- The right vas is brought into the wound with its vasectomy site.
- Babcock clamps go above and below the obstructed segment and a Penrose drain through an avascular plane in its sheath.
- 04
Freeing the ends, square cuts
- Under the microscope the vas is freed from scar, its vasal artery kept.
- Each end is held in a slotted nerve clamp and cut cleanly at a right angle beyond the scar; the scarred segment comes out.
- 05
Testicular fluid and abdominal patency
- Fluid from the testicular end goes on a slide: sperm present, so the plan is a vasovasostomy.
- The abdominal end is dilated, cannulated and irrigated: it is patent.
- 06
Anterior wall: mucosa, then muscularis
- The ends in an approximating clamp, the six mucosal sites marked.
- Three mucosal stitches on the anterior wall, placed inside out, then three deep muscular stitches between them.
- 07
Half turn, posterior wall, sheath
- The vas is turned a half turn and irrigated, and the posterior wall done the same way, mucosa then muscularis.
- The sheath is closed over the join, which lies without tension.
- 08
The other side
- The right vas goes back joined.
- The left vas is delivered, the scar cut out, its fluid checked and joined the same way in two layers.
- 09
Closure and follow-up
- Haemostasis, the dartos and the skin closed with absorbable stitches, a dressing and a support.
- Semen analysis follows; patency and pregnancy are reported separately.
Derived from scrotal/VasectomyReversalScene.tsx at build time: every step title and caption above is the authored
ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation
itself. 9 of 9 steps have a captured frame.
Text fingerprint ade130f01b213835. A caption edit without a re-capture fails the production
check, so the sheet and the animation cannot drift apart silently.
