Adult acquired buried penis reconstruction

9 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.

Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

    Assess and optimise

Adult acquired buried penis reconstruction. Step 1: Assess and optimise. Captured frame from the interactive animation.
    • A large pannus hangs over a deep suprapubic fat pad; only a moist, narrowed opening shows and the glans is hidden.
    • Seen through the fat, the shaft is of normal length.
    • Lichen sclerosus has scarred its skin, worst ventrally.
    • Weight, diabetes and the skin disease are addressed first.
  2. 02

    Position and mark

Adult acquired buried penis reconstruction. Step 2: Position and mark. Captured frame from the interactive animation.
    • Supine, the pannus held up.
    • A transverse ellipse is marked over the mons: its lower edge just above the penile base, its upper edge on the abdomen, wide enough to take the fat pad that buries the shaft.
  3. 03

    Release the cicatrix

Adult acquired buried penis reconstruction. Step 3: Release the cicatrix. Captured frame from the interactive animation.
    • The cicatricial ring at the mouth of the pit is divided at 12 o'clock and the mouth opens.
    • The glans comes into view and a stay stitch through it holds the shaft up for the rest of the operation.
  4. 04

    Escutcheonectomy

Adult acquired buried penis reconstruction. Step 4: Escutcheonectomy. Captured frame from the interactive animation.
    • The ellipse is incised and the wedge of skin and fat is taken off the rectus fascia en bloc.
    • The lymphatic-rich tissue at the lateral margins is divided with care.
    • With the fat pad gone, the penis stands clear of the abdomen.
  5. 05

    Excise the diseased shaft skin

Adult acquired buried penis reconstruction. Step 5: Excise the diseased shaft skin. Captured frame from the interactive animation.
    • The scarred shaft skin is excised from the corona to the base, superficial to Buck's fascia.
    • The dorsal neurovascular bundle lies beneath the fascia and the urethra just under the ventral skin.
  6. 06

    Fixation and closure

Adult acquired buried penis reconstruction. Step 6: Fixation and closure. Captured frame from the interactive animation.
    • The dermis at the penile base is anchored to the rectus fascia so the penopubic angle is held and the penis cannot be drawn back into the fat.
    • The abdominal flap is advanced and the wound closed transversely.
  7. 07

    Split skin graft

Adult acquired buried penis reconstruction. Step 7: Split skin graft. Captured frame from the interactive animation.
    • A split skin graft taken from the thigh is laid on the bare shaft, wrapped round it with the seam ventral, sutured at the corona and the base and quilted to the shaft so it cannot lift or shear.
  8. 08

    Dressing and catheter

Adult acquired buried penis reconstruction. Step 8: Dressing and catheter. Captured frame from the interactive animation.
    • A compressive dressing holds the graft still against its bed; a urethral catheter keeps it dry.
    • The graft, the abdominal wound, the donor site and the voiding are watched until the dressing comes down.
  9. 09

    The later result

Adult acquired buried penis reconstruction. Step 9: The later result. Captured frame from the interactive animation.
    • The graft has taken and the penis stands clear of a flat suprapubic area with a transverse scar.
    • He can stand to void and keep clean.
    • Keeping the weight off is what keeps it unburied.

Derived from BuriedPenisScene.tsx at build time: every step title and caption above is the authored ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation itself. 9 of 9 steps have a captured frame.

Text fingerprint 738ef91fe73c51da. A caption edit without a re-capture fails the production check, so the sheet and the animation cannot drift apart silently.
