Revision sheet
The forgotten stent
6 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.
Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

Encrustation burden on imaging
- A plain film with a cross-sectional map: every encrusted segment of the stent is located before any instrument is picked up.
- The question imaging answers is where is it free, and where is it welded: the strategy follows the answer.
- 02
Infection settled first
- Culture before instrumentation, every time.
- An obstructed infected system behind an encrusted stent is drained and treated before clearance is attempted: opening a closed infection under pressure is how a stent problem becomes a septic problem.
- 03
Cystoscopy + laser decrustation
- The scope reaches the bladder coil and the laser fibre peels its calcification, one deposit at a time, until the coil is soft enough to move.
- Decrustation first, traction second: the order is not negotiable.
- 04
No pull; ureteroscopy for the shaft
- Graspers hold the freed coil, but the renal coil is welded, so no traction: the ureter must never be the thing that yields.
- A semirigid ureteroscope then clears the shaft crust alongside the stent, level by level.
- 05
Combined clearance for the heavy coil
- The same anaesthetic continues: a straight posterolateral tract into a posterior lower calyx, a flexible nephroscope decrusts the renal coil and the upper ureter, then graspers through the cystoscope bring the whole stent out through the ureter and urethra in one piece.
- 06
The register
- The operation that ends this case started months ago as an entry: removal date recorded at insertion, patient diary, stent register.
- Every forgotten stent is a systems failure, not a patient failure.
Derived from ForgottenStentScene.tsx at build time: every step title and caption above is the authored
ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation
itself. 6 of 6 steps have a captured frame.
Text fingerprint e9796dea84b444d5. A caption edit without a re-capture fails the production
check, so the sheet and the animation cannot drift apart silently.
