Revision sheet
Simple prostatectomy (Millin retropubic)
9 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.
Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

- 01
Who
- The gland over eighty or a hundred grams, the bladder with a stone or a diverticulum, the man the scope could not finish.
- Laser enucleation does the same job through the urethra where it is available.
- 02
Open retropubic, transvesical or robotic
- Retropubic (Millin) through the capsule, the operation shown here; transvesical (Freyer) through the bladder for the big median lobe or the stone; robotic through either.
- Cross-match, a catheter, and the warning about retrograde ejaculation and bleeding.
- 03
Exposure and the dorsal venous complex
- Extraperitoneal, the fat swept off the anterior prostate and bladder neck, a self-retaining retractor set.
- The dorsal venous complex stitched above and below the line of the capsulotomy first.
- 04
The capsulotomy
- Transverse, a centimetre below the bladder neck, through the capsule into the adenoma, the lips held on stay sutures.
- The plane is found when the adenoma shows: pale, lobulated, distinct from the thin capsule around it.
- 05
The enucleation plane
- A finger in the plane, sweeping the adenoma off the capsule laterally and posteriorly, each lobe in turn.
- Stay in the plane: outside it is the capsule and the venous plexus.
- 06
The apex and the sphincter
- At the apex the adenoma is pinched off the urethra, not torn: the sphincter lies just beyond.
- The urethral mucosa divided sharply under vision and the adenoma delivered.
- 07
Haemostasis and trigonisation
- The bed packed, then figure-of-eight sutures at five and seven o'clock at the neck where the arteries enter, and the posterior neck advanced into the bed so the fossa is covered and the neck does not contract.
- 08
Capsule closure, catheter, drain
- A three-way catheter with its balloon in the bladder, not the fossa; the capsule closed with a running suture over it; a drain in the retropubic space; irrigation started on the table.
- 09
After
- Irrigation until clear, the catheter five to seven days, the drain out when dry.
- The stream is transformed; leakage settles over weeks; the neck watched for the contracture that comes to a few.
Derived from pelvis/SimpleProstatectomyScene.tsx at build time: every step title and caption above is the authored
ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation
itself. 9 of 9 steps have a captured frame.
Text fingerprint 3a762cbe49820a6a. A caption edit without a re-capture fails the production
check, so the sheet and the animation cannot drift apart silently.
