Female urethral diverticulectomy

9 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.

Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

  1. 01

    Catheter, speculum, infiltration

Female urethral diverticulectomy. Step 1: Catheter, speculum, infiltration. Captured frame from the interactive animation.
    • Lithotomy.
    • A urethral catheter is passed first and the bladder is emptied.
    • A weighted speculum draws the posterior wall back and a self-retaining retractor holds the labia.
    • Local anaesthetic with adrenaline is infiltrated under the anterior vaginal wall, to open the plane and to limit bleeding.
  2. 02

    Inverted-U flap

Female urethral diverticulectomy. Step 2: Inverted-U flap. Captured frame from the interactive animation.
    • With the bladder empty, an inverted-U incision is made in the anterior vaginal wall, from just proximal to the meatus back to the level of the proximal urethra.
    • Sharp and blunt dissection raises the flap of vaginal wall off the periurethral fascia beneath it.
  3. 03

    Open the periurethral fascia

Female urethral diverticulectomy. Step 3: Open the periurethral fascia. Captured frame from the interactive animation.
    • The periurethral fascia is incised transversely and dissected carefully off the sac, superiorly and inferiorly, as two leaves.
    • The sac itself is not entered.
  4. 04

Free the sac to its neck

Female urethral diverticulectomy. Step 4: Free the sac to its neck. Captured frame from the interactive animation.
    • The sac is grasped with a Babcock clamp and dissected free all the way round, down to its origin on the urethra.
  5. 05

    Excise the sac

Female urethral diverticulectomy. Step 5: Excise the sac. Captured frame from the interactive animation.
    • The whole sac is excised at its urethral origin.
    • The urethral catheter is then seen through the opening in the ventral wall of the urethra.
  6. 06

    Close the urethra

Female urethral diverticulectomy. Step 6: Close the urethra. Captured frame from the interactive animation.
    • The urethra is reconstructed over the catheter with interrupted absorbable stitches placed in a row along it: a closure that is watertight and free of tension.
  7. 07

    Close the fascia across

Female urethral diverticulectomy. Step 7: Close the fascia across. Captured frame from the interactive animation.
    • The two leaves of periurethral fascia are closed transversely with a running absorbable stitch, at right angles to the urethral closure, to limit overlap of the suture lines.
    • Bleeding points are controlled with sparing diathermy.
  8. 08

    Cystourethroscopy

Female urethral diverticulectomy. Step 8: Cystourethroscopy. Captured frame from the interactive animation.
    • The catheter is removed and cystourethroscopy is done: the proximal urethra and the bladder are intact and there is clear efflux from both ureteric orifices.
    • The scope is removed and the catheter replaced.
  9. 09

    Close the vaginal wall

Female urethral diverticulectomy. Step 9: Close the vaginal wall. Captured frame from the interactive animation.
    • The flap is laid back and the anterior vaginal wall is closed with interrupted absorbable stitches as the third layer.
    • The retractor and the speculum are removed and the catheter is left on drainage.

Derived from female/UrethralDiverticulectomyScene.tsx at build time: every step title and caption above is the authored ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation itself. 9 of 9 steps have a captured frame.

Text fingerprint 0dabd7f9384c41fd. A caption edit without a re-capture fails the production check, so the sheet and the animation cannot drift apart silently.
