Renal vein or artery thrombosis
- the kidney that stops making urine in the first days, or never starts: a kinked or twisted vessel, a stretched vein, an intimal flap, a hypotensive night or a hypercoagulable recipient.
- Check the catheter and flush it, then a duplex ultrasound within the hour; absent flow or reversed diastolic flow is theatre now. first move
- Tell the transplant consultant and the anaesthetist; the window for salvage is measured in minutes to an hour or two.
- Re-explore: the vessels inspected for kink, twist and thrombus, a thrombectomy and revision of the anastomosis if the kidney is salvageable.
- A kidney that is black, soft and does not reperfuse after thrombectomy: graft nephrectomy, and the patient back to dialysis. escalation trigger
- Op note: the time the urine stopped, the duplex findings, the vessel involved, what was done and the outcome for the graft. document
