Immediate intravesical chemotherapy

8 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.

Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

  1. 01

    Why

Immediate intravesical chemotherapy. Step 1: Why. Captured frame from the interactive animation.
    • Cells shed at the resection float in the urine and implant on the raw mucosa.
    • A single dose within 24 hours kills them and reduces recurrence in low and intermediate risk disease; it does nothing for tumour left behind.
  2. 02

    Who does not get it

Immediate intravesical chemotherapy. Step 2: Who does not get it. Captured frame from the interactive animation.
    • A perforation seen or suspected, fat in the base, a very extensive resection, or bleeding that needs irrigation.
    • Mitomycin in the perivesical fat or the peritoneum is a chemical burn that declares itself over weeks.
  3. 03

At the end of the TURBT

Immediate intravesical chemotherapy. Step 3: At the end of the TURBT. Captured frame from the interactive animation.
    • The bladder inspected once more with the irrigation off: base dry, wall intact.
    • The scope out, a catheter in, and the bladder empty, or the dose is diluted to nothing.
  4. 04

    The dose

Immediate intravesical chemotherapy. Step 4: The dose. Captured frame from the interactive animation.
    • Mitomycin C 40 mg in 40 ml of water, or the local agent, made up by pharmacy and handled as cytotoxic: gloves, gown, eye protection, a spill kit within reach.
  5. 05

    Instillation and the hour

Immediate intravesical chemotherapy. Step 5: Instillation and the hour. Captured frame from the interactive animation.
    • Instilled slowly through the catheter and the catheter clamped for one hour, the patient turned a quarter every fifteen minutes so the drug reaches every wall.
  6. 06

    Drain and protect

Immediate intravesical chemotherapy. Step 6: Drain and protect. Captured frame from the interactive animation.
    • Unclamped and drained, the catheter flushed with saline, the skin washed, bag and syringe into cytotoxic waste, and the urine treated as cytotoxic for six hours.
  7. 07

    The catheter and the ward

Immediate intravesical chemotherapy. Step 7: The catheter and the ward. Captured frame from the interactive animation.
    • The catheter stays or goes as the resection decides.
    • Dysuria for a day or two is the drug; pain that grows is not.
  8. 08

    The recurrence pathway

Immediate intravesical chemotherapy. Step 8: The recurrence pathway. Captured frame from the interactive animation.
    • Histology decides the risk group and the risk group the surveillance: cystoscopy at three months for everyone, a course of chemotherapy or BCG for intermediate risk, BCG for high, surveillance alone for low.

Derived from endoscopy/ImmediateChemoScene.tsx at build time: every step title and caption above is the authored ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation itself. 8 of 8 steps have a captured frame.

Text fingerprint cd2c2b318e09bc4e. A caption edit without a re-capture fails the production check, so the sheet and the animation cannot drift apart silently.
