Emergency decompression of the infected obstructed kidney
9 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.
Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

- 01
Who
- A stone in the ureter, a fever, a pulse over a hundred and a pressure that is falling: infection above an obstruction.
- Also the tumour, the stricture and the pregnancy that obstruct; the cause does not change the rule.
- 02
Sepsis six and the clock
- Oxygen, cultures, antibiotics, fluids, lactate, a catheter within the hour, and the call to drain at the same time, not after.
- The pus must come out tonight, and the clock is the patient's pressure.
- 03
Imaging: CT without contrast
- The stone, its size and level, the hydronephrosis, the fat stranding, and gas that means an emphysematous kidney.
- The creatinine and potassium, the other kidney, the clotting and antiplatelets that decide between needle and scope.
- 04
Nephrostomy or stent
- Both drain.
- The nephrostomy under local for the sick patient, the big stone, the pus that will not pass a stent; the stent under anaesthetic for the small distal stone, the anticoagulated, the pregnant.
- The choice is who is available and how sick the patient is.
- 05
The nephrostomy
- Prone, local anaesthetic to the capsule under ultrasound, the needle into the posterior lower calyx through Brodel's line below the twelfth rib;
- urine or pus back and sampled;
- the wire coiled in the pelvis, the tract dilated, an 8 French pigtail locked in.
- 06
The alternative: a retrograde stent
- Instead of the nephrostomy, not as well: cystoscopy, the orifice, a wire past the stone under screening, a stent over it, and no attempt at the stone tonight.
- A wire that will not pass is a nephrostomy on the same table.
- 07
Pus
- It drains, it is cultured, the antibiotics are changed to what grows, and the tube stays until the fever and counts settle.
- Gas in the kidney or a kidney that does not settle is emphysematous pyelonephritis: drainage, and a nephrectomy for the one that fails.
- 08
The stone later
- Treated weeks later when the infection is gone, by ureteroscopy or lithotripsy, with the tube or the stent still in until it is.
- 09
After
- An obstructed infected kidney is drained the night it is found, and the register records the time from door to drain.
Derived from kidney/InfectedObstructedKidneyScene.tsx at build time: every step title and caption above is the authored
ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation
itself. 9 of 9 steps have a captured frame.
Text fingerprint d0d06befc90f6d5a. A caption edit without a re-capture fails the production
check, so the sheet and the animation cannot drift apart silently.
