    Ultrasound survey

    • Prone position, ultrasound on: confirm the dilated system, pick a posterior calyx below the 12th rib, and clear the colon off the intended line before the needle is loaded.
  2. 02

    Calyceal puncture + aspiration

    • The needle enters the calyx through the parenchyma along the infundibular axis.
    • Aspirate urine for culture - efflux confirms position; no efflux means re-image, not push.
  3. 03

    Wire (Seldinger)

    • A guidewire coils into the pelvis.
    • Enough wire must sit inside the system that it cannot fall out during exchange.
  4. 04

    Tract + pigtail drain

    • Dilate over the wire and railroad the pigtail drain into the pelvis.
    • The coil, not the tip, holds the drain in the system.
  5. 05

    Secure + check

    • Aspirate again, sample for culture, secure the drain to skin and confirm position with a film or CT.
    • Decompression, not stone clearance, is the operation.
