  1. 01

    Who gets a balloon

    • 30 to 80 g with lateral lobe obstruction, in a man who wants ejaculation kept and one procedure rather than a week with a device in.
    • Consent covers a catheter for two to three days, blood, urgency, infection, and a coating whose long-term effect is still being followed.
  2. 02

    The room and the map

    • General or spinal anaesthetic, antibiotics, cystoscopy first.
    • The veru is the distal limit.
    • The length from the neck to the veru picks the balloon (typically 30 x 30 or 30 x 35 mm), and a wire goes into the bladder.
  3. 03

    Anchor, then lock

    • The pre-dilation catheter goes over the wire.
    • The distal lobe inflates in the bladder as the anchor, and the reinforced neck is pulled back until it locks on the bladder neck.
    • It cannot migrate while it is inflated.
  4. 04

    The commissurotomy

    • The proximal lobe dilates inside the prostatic urethra until the anterior commissure gives way at twelve o'clock.
    • That split is the operation: nothing is resected and no current is used.
    • Never at six, where the veru and the ejaculatory ducts are.
  5. 05

    The coated balloon

    • The pre-dilation balloon comes down and the coated balloon goes up the same wire, seated the same way.
    • Inflated and held, typically at least five minutes, so paclitaxel transfers into the raw surface of the split and the lobes do not fuse back.

    The end of the case

    • Down, out, and a look: the channel open from the neck to the veru and the split anterior.
    • A catheter for two to three days, because the split surface bleeds and swells before it settles.
  7. 07

    What it leaves behind

    • An anterior channel held open by a split that has not fused back, with the lobes themselves untouched.
    • Symptoms improve early and ejaculation is usually kept.
    • Durability past the trial years is the open question, and the man is told so.
