Revision sheet
Rectocele repair (posterior colporrhaphy)
8 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.
Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

- 01
The bulge
- Lithotomy, a catheter in the bladder.
- A retractor lifts the anterior wall and a self-retaining retractor holds the introitus open.
- The posterior vaginal wall bulges forward because the anterior rectal wall is pushing it: only the rectovaginal septum lies between them.
- 02
The triangle
- The posterior wall is infiltrated.
- An Allis clamp goes on each lateral margin at the posterior fourchette and a third in the midline at the upper border of the rectocele.
- Between them a triangle of epithelium is incised, its base at the fourchette.
- 03
Dissection
- Scissors lift the epithelium off the perirectal fascia, away from the rectum.
- A finger in the rectum can show how close its anterior wall is.
- With the rectum drawn downward, the dissection is carried lateral to the bulge until the rectovaginal fascia is exposed on each side.
- 04
Plication sutures
- Interrupted sutures are placed in the rectovaginal fascia, starting at the apex and working down to the fourchette.
- The bites are shallow, so the rectum is not taken, and the levator ani is left out.
- All are placed before any is tied.
- 05
Tie
- The sutures are tied one at a time, in succession.
- The fascia comes together in the midline over the rectum and the bulge goes.
- 06
Perineorrhaphy
- Added only where it is indicated, as for this attenuated perineal body.
- A second triangle, of perineal and vaginal skin at the posterior commissure, makes the defect a diamond.
- Bulbocavernosus and the transverse perineal muscles are brought together in the midline with vertical sutures.
- 07
Calibre
- Before the incisions are closed the calibre is checked.
- The vagina should take at least two fingers in a sexually active woman: a repair drawn too tight is a painful one.
- 08
Closure and rectal examination
- The edges of the epithelium are trimmed and the midline incision is closed with a running suture, then the perineal skin.
- Last, a finger in the rectum confirms an intact wall, no bleeding and no stitch to feel.
Derived from female/PosteriorColporrhaphyScene.tsx at build time: every step title and caption above is the authored
ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation
itself. 8 of 8 steps have a captured frame.
Text fingerprint 418e7e001598fcd7. A caption edit without a re-capture fails the production
check, so the sheet and the animation cannot drift apart silently.
