Revision sheet
Robot-assisted radical prostatectomy (nerve-sparing)
7 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.
Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

Drop the bladder: space of Retzius
- Robot docked, 8 mm cannulas, steep head-down.
- The retropubic fat is swept off the pubis and prostate to expose the endopelvic fascia.
- Landmarks fixed: puboprostatic ligaments, the deep dorsal venous complex, the catheter in the urethra.
- 02
Bladder neck dissection
- The plane between bladder and prostate is opened at 12 then around.
- The neck is transected exactly at the prostate: too wide damages the ureteric orifices, too narrow turns into the prostate.
- 03
Seminal vesicles + vasa
- Behind the neck, the vasa are clipped and divided and the seminal vesicles freed.
- The vesicle tips mark where Denonvilliers' fascia is entered next.
- Seen here as a cutaway from behind.
- 04
Denonvilliers’ plane
- The pearly plane between the prostate-vesicle packet and the rectum is opened and developed to the apex.
- Stay on the fascia; the rectum is the next layer down.
- 05
Neurovascular bundles released
- The prostatic pedicles are clipped at the base and cut without thermal energy beside the bundles; the bundles peel off the prostate posterolaterally and are left in the pelvis.
- Sacrifice is a cancer decision, not an accident.
- 06
DVC + urethra at the apex
- The dorsal venous complex is controlled and divided, then the urethra is cut at the apex with as much sphincter and urethral length as the cancer allows: the continence and anastomosis live on this stump.
- The specimen goes into a bag.
- 07
Vesicourethral anastomosis
- A posterior row first, the bladder neck parachuted down onto the stump, a new catheter, then the anterior row: a running anastomosis, tested watertight, and a drain left.
- The anastomotic leak and the bladder-neck contracture are both born here.
Derived from RadicalProstatectomyScene.tsx at build time: every step title and caption above is the authored
ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation
itself. 7 of 7 steps have a captured frame.
Text fingerprint be97f972c9b76c56. A caption edit without a re-capture fails the production
check, so the sheet and the animation cannot drift apart silently.
