  1. 01

    Midline exposure of the retroperitoneum

    • The midline is opened and the posterior peritoneum incised, the duodenum and right colon mobilised off the great vessels until the retroperitoneum sits bare from renal veins to the bifurcation.

    Template boundaries

    • The template is chosen before a vessel is touched: full bilateral for post-chemotherapy residual masses and bulky or bilateral disease, modified unilateral only for selected primary cases.

    Dissection on the aorta

    • The aorta is lifted and rolled and the lumbar arteries ligated or clipped as the packet comes off its wall.
    • The inferior mesenteric artery is preserved unless the template demands its division.

    • The cava is rolled the same way.
    • The lumbar veins are the named pitfall of the operation: short, posterior, and they tear the cava at its back wall if pulled instead of controlled.
  5. 05

    Sympathetic chain preservation

    • The sympathetic chains are identified running longitudinal and lateral, picked up with the golden fibres of the postganglionic nerves, and preserved when the disease allows.
    • Ejaculation survives only if they do.
  6. 06

    Residual mass out intact

    • The mass and all packets leave the vessel walls clean and intact.
    • Teratoma does not respond to chemotherapy: what is left behind grows, and malignant transformation is its endgame.
