Graft vascular injury and haemorrhage
- the graft artery or vein entered during a nephrostomy, an antegrade incision or a reoperative dissection: haematuria through the nephrostomy, a haematoma compressing the graft, or bleeding in the field.
- Pressure on the field or the nephrostomy clamped, the patient resuscitated and cross-matched, and the transplant surgeon called. first move
- Duplex ultrasound for the perfusion of the graft and the size of the haematoma; a graft with no flow is theatre now.
- Bleeding from the nephrostomy tract that does not settle with a larger tube: angiography and selective embolisation.
- A torn graft vessel in the field, or a graft losing perfusion: vascular help, repair under control, and graft nephrectomy if the iliac artery is threatened. escalation trigger
- Op note: the vessel, the control, the transfusion and the perfusion of the graft at the end. document
