Penile fasciocutaneous flap urethroplasty
8 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.
Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

- 01
Mark the island
- A stitch through the glans puts the penis on stretch.
- Under loupes the skin is marked all the way round a short distance proximal to the corona, and again a measured distance proximal to it, outlining the band.
- The band between the lines is the flap: hairless distal penile skin.
- 02
Distal incision
- The distal line is cut through skin, dartos and the superficial lamina of Buck's fascia.
- Those layers go with the island as its pedicle.
- The deep layer of Buck's fascia stays on the corpora with the dorsal bundle beneath it.
- 03
Proximal incision and deglove
- The proximal line is cut through skin only, and the shaft skin is taken back to the penoscrotal junction above the dartos.
- The subdermal plexus stays on the skin; the dartos stays with the island.
- The result is a ring of skin on a broad pedicle.
- 04
Divide and transpose
- The ring and its pedicle are divided sharply in the midline, ventrally here; the side is chosen to keep the better blood supply.
- The strip is turned through a right angle and laid beside the urethra without tension.
- 05
Urethrotomy
- A catheter is passed to the stricture and the urethra opened on its tip in the ventral midline.
- The cut runs proximally through the whole stricture into healthy urethra.
- Stay sutures hold the edges and the urethra is calibrated with bougies.
- 06
Tailor, turn over, first edge
- The defect is measured and the island tailored to fit it.
- It is turned over on to the opened urethra, skin to the lumen, and its pedicle edge is run to the border of the corpus spongiosum on that side.
- 07
Apices and free edge
- Interrupted full-thickness stitches fix the proximal and distal apices to the ventral wall of the spongiosum.
- A catheter is passed to the bladder before the repair is finished, and the free edge is run to the other border.
- The pedicle then lies over the repair.
- 08
Skin closure
- The repair is checked for tension and bleeding.
- The shaft skin is advanced and sewn to the subcoronal edge: four quadrant stitches, then interrupted stitches between.
- A loose dressing; the catheter on drainage.
Derived from PenileFlapUrethroplastyScene.tsx at build time: every step title and caption above is the authored
ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation
itself. 8 of 8 steps have a captured frame.
Text fingerprint f35b75df9bb5259e. A caption edit without a re-capture fails the production
check, so the sheet and the animation cannot drift apart silently.
