Revision sheet

Radical cystectomy + ileal conduit

7 steps, each one the authored title and caption from the interactive animation, with the frame captured from that step. Read it before the case, or print it and work through the steps in order.

Teaching only, not for diagnosis or treatment decisions. The text on this sheet is the animation's own step copy, reproduced verbatim.

  1. 01

    Access + stoma site

Radical cystectomy + ileal conduit. Step 1: Access + stoma site. Captured frame from the interactive animation.
    • The stoma site is marked before induction with the patient standing, over the rectus and clear of the belt line.
    • A midline (or robotic) incision then opens the abdomen.
  2. 02

    Ureteric division

Radical cystectomy + ileal conduit. Step 2: Ureteric division. Captured frame from the interactive animation.
    • Both ureters are skeletonised at the pelvic brim and divided between clips.
    • The ureter crosses the iliac bifurcation directly over the vessels: traction and blind clips are what injure the iliac veins.
  3. 03

    Posterior plane

Radical cystectomy + ileal conduit. Step 3: Posterior plane. Captured frame from the interactive animation.
    • The plane is opened behind the vas and seminal vesicles onto Denonvilliers' fascia and dropped to the apex.
    • It is avascular until a wrong-layer entry puts the scalpel on the rectal wall.
  4. 04

    Lateral pedicles

Radical cystectomy + ileal conduit. Step 4: Lateral pedicles. Captured frame from the interactive animation.
    • The lateral bladder pedicles are controlled along the internal iliac branches, stepping from the divided ureter down toward the apex.
    • The obturator nerve shares the same lateral gutter.
  5. 05

    Apex + dorsal venous plexus

Radical cystectomy + ileal conduit. Step 5: Apex + dorsal venous plexus. Captured frame from the interactive animation.
    • The dorsal venous plexus is controlled and divided at the apex with the urethra.
    • Torn, it retracts into the pelvic floor: the save is a stitch, never blind diathermy.
  6. 06

    Lymphadenectomy

Radical cystectomy + ileal conduit. Step 6: Lymphadenectomy. Captured frame from the interactive animation.
    • The node template runs from the aortic bifurcation or common iliacs to the inguinal ligament, including the obturator packets, matched to the disease and the patient.
  7. 07

    Ileal conduit + stoma

Radical cystectomy + ileal conduit. Step 7: Ileal conduit + stoma. Captured frame from the interactive animation.
    • About 15 cm of ileum at least 15 cm from the caecum is selected on a healthy mesentery and bowel continuity restored above it.
    • The conduit lies isoperistaltic: both ureters join its proximal (butt) end end to side (Bricker) over 8 Fr stents, the left brought under the sigmoid mesentery, and the distal end is matured through the marked right-sided site as a spouted bud.

Derived from RadicalCystectomyScene.tsx at build time: every step title and caption above is the authored ProcedureDef text, reproduced verbatim, and this page shares its readable-points renderer with the animation itself. 7 of 7 steps have a captured frame.

Text fingerprint 5a01a9870c4015e4. A caption edit without a re-capture fails the production check, so the sheet and the animation cannot drift apart silently.
