Outlet-reduction surgery in neuro-urology
Procedures that intentionally lower bladder-outlet resistance to improve low-pressure emptying in selected neuro-urological patients.

Typical place in practice
A carefully selected high-resistance outlet when catheterisation is not feasible or preferred and continuous low-pressure drainage into a device is acceptable.
Alternatives and neighbouring procedures
Intermittent or indwelling catheterisation, pharmacological management, botulinum toxin to the sphincter, diversion or other reconstructive strategies.
Same procedure family, explicit technique branches
These are variations of this canonical operation. Complication modules below identify whether they apply to all branches or only to named approaches.
External sphincterotomy
Permanently reduces external sphincter resistance.
Bladder-neck incision or resection
Targets bladder-neck resistance in selected anatomy.
Urethral stent pathway
Maintains patency with an implant; use is limited and device-specific.
High-level sequence
Coverage outline only; not a step-by-step technical guide.
- Document pressure, outlet function and drainage goals
- Identify the anatomical level of resistance
- Reduce resistance in a controlled endoscopic plane
- Confirm drainage and exclude significant injury
- Monitor upper tracts, residual pressure, infection and continence-device needs
Every failure mode is its own module.
Timing and variation coverage are recorded for each module.
Bleeding or capsular/peri-urethral injury
Deep incision or resection can enter vascular tissue or create extravasation.
Applies to: All variantsUrinary infection
Instrumentation and subsequent drainage arrangements can increase infection risk.
Applies to: All variantsInadequate pressure reduction or recurrent obstruction
Scar, residual sphincter activity or incorrect level selection may leave emptying unsafe.
Applies to: All variantsExpected or worsened urinary incontinence
Continence loss is inherent to outlet reduction and must align with the planned collection strategy.
Applies to: All variantsErectile or ejaculatory change
Treatment close to erectile and ejaculatory structures can alter sexual function.
Applies to: External sphincterotomy · Bladder-neck incision or resectionMigration, encrustation or erosion
An outlet stent can move, become encrusted, obstruct or injure urethral tissue.
Applies to: Urethral stent pathwaySources for this module
Botulinum toxin, sphincterotomy, neuromodulation, augmentation and diversion.
